Align Cuts 2026 Revenue Outlook as Scanner Sales Weigh
Align Technology reported third-quarter revenue rose 1.9% year over year to $1.015 billion, driven by Clear Aligner volume growth, while profitability improved significantly with GAAP operating margin reaching 16.7%. Despite strong case shipments and margin expansion, the company lowered its 2026 revenue outlook due to a shift toward lower-priced scanners and flexible acquisition models weighing on Systems and Services sales. Management forecasts continued volume growth, at least 100 basis points of non-GAAP operating-margin improvement for 2027, and ongoing share repurchases, while navigating risks including soft discretionary healthcare spending and foreign-exchange pressures.
Align Technology reported higher third-quarter revenue and stronger profitability, supported by Clear Aligner volume growth, but reduced its 2026 revenue outlook as the shift toward lower-priced scanners and flexible acquisition models weighed on Systems and Services sales. The company also slightly lowered its Clear Aligner volume and average selling price assumptions, citing product and geographic mix as well as foreign exchange.
Revenue for the quarter ended September 30 rose 1.9% year over year to $1.015 billion, or 2.4% on a constant-currency basis. Revenue declined 3.9% sequentially. Clear Aligner revenue increased 4.2% to $839.3 million, while Systems and Services revenue fell 7.6% to $175.5 million.
Profitability improved more sharply. GAAP operating margin reached 16.7%, up 7.0 percentage points from a year earlier, while non-GAAP operating margin increased 0.5 percentage points to 24.4%. GAAP operating income rose to $169.2 million from $96.3 million, and non-GAAP operating income increased by $10.0 million to $247.8 million.
GAAP net income was $135.0 million, or $1.90 per diluted share, compared with $56.8 million, or $0.78 per share, a year earlier. Non-GAAP net income rose to $198.9 million from $189.0 million, with diluted earnings per share increasing to $2.80 from $2.61. Align said lower excluded adjustments also contributed to the improvement in GAAP results.
Clear Aligner volumes rise across age groups
Clear Aligner case shipments increased 6.6% year over year to about 690,600, nearly unchanged from the second quarter. The number of submitting doctors rose 3.1% to 90,900, while doctor utilization increased 3.4% to 7.6 cases per doctor.
Revenue per case was $1,215, down 2.3% year over year on a reported basis but up 1.8% at constant currency. Align said the result was consistent with its expectations.
Shipments increased across patient age groups. Teen and children’s shipments rose 7.3% year over year and 14.8% sequentially to approximately 275,000. Adult shipments increased 6.2% from a year earlier but fell 8.1% from the second quarter to about 416,000.
Regional performance was led by EMEA and Asia-Pacific retail demand. EMEA retail Clear Aligner shipments rose 8%, with total shipments and dental support organization volumes growing at double-digit rates. Asia-Pacific retail shipments increased 16%, supported by record doctor submitters and double-digit teen growth led by China.
Americas Clear Aligner shipments increased 1.5%. North America recorded its first year-over-year growth in orthodontist shipments in six quarters, while DSO shipments rose at a double-digit rate. Align said retail demand remained pressured, particularly among general practitioners, although orthodontist retail shipments improved sequentially. Latin America generated double-digit growth across channels.
The company is using patient financing, doctor subscription programs, peer mentoring and treatment-planning services to address barriers to case starts. It said shipments through its Doctor Subscription Program grew at a double-digit rate, while shipments under its lower-upfront-price “No AA” model increased at a triple-digit rate.
Flexible scanner models pressure reported revenue
Systems and Services revenue decreased 7.6% year over year and 5.3% sequentially to $175.5 million. Align attributed the reported decline to a deliberate shift toward leasing, rental and other flexible acquisition models rather than weaker demand. Leased scanner units grew at a double-digit rate, and the installed base exceeded 157,000 units, including leased, rented and certified pre-owned systems.
The company said like-for-like scanner pricing stabilized during the quarter, but product mix continued to move toward lower-priced scanners and flexible acquisition options. These models are intended to reduce customers’ initial cost of adopting Align’s digital technology, though they can defer revenue recognition.
Full-year revenue outlook reduced
Align now expects 2026 worldwide revenue of $4.11 billion to $4.13 billion. Management said the updated range is below its prior outlook, primarily because of lower expected fourth-quarter Systems and Services revenue and slightly reduced Clear Aligner volume and ASP assumptions.
The company continues to expect full-year Clear Aligner volume and revenue growth of approximately 5% to 6%, with ASP slightly below 2025. Systems and Services revenue is projected at $715 million to $725 million.
Align expects a 2026 GAAP operating margin of approximately 16.2%, an increase of about 2.7 percentage points year over year. Its non-GAAP operating-margin forecast remains approximately 23.7%, representing an improvement of 100 basis points. Planned capital expenditures are $125 million to $150 million.
For the fourth quarter, Align forecast revenue of $1.00 billion to $1.02 billion. Clear Aligner volume is expected to increase at a low-single-digit rate year over year, while Clear Aligner ASP is projected to decline about 1%. Systems and Services revenue is expected to range from $170 million to $180 million. The company anticipates a GAAP operating margin of about 20% and a non-GAAP operating margin of approximately 26%.
Management said discretionary healthcare spending and capital-equipment investment remained soft during the third quarter and that the fourth-quarter outlook reflects continued pressure across the broader dental and orthodontic market.
Preliminary 2027 framework targets further margin expansion
Align’s preliminary planning framework calls for revenue growth and another year of operating-margin expansion in 2027, based on an operating environment broadly consistent with current conditions and without assuming a consumer recovery.
The company expects continued Invisalign volume growth and broadly stable Clear Aligner pricing, with ASPs slightly lower because of product and geographic mix. For Systems and Services, management expects unit growth, a larger installed base and stable ASPs to support revenue growth in 2027.
Align is also targeting at least 100 basis points of non-GAAP operating-margin improvement in 2027 while maintaining investment in growth initiatives. Full 2027 guidance is scheduled to be provided with fourth-quarter results.
The company generated $183.2 million of operating cash flow in the third quarter and $138.2 million of free cash flow after $45.0 million of capital spending. Cash and cash equivalents totaled $1.145 billion. Align repurchased $103.3 million of shares during the quarter and $200 million through the first nine months. It plans to repurchase about $500 million in 2026, including up to $300 million in the fourth quarter.
The outlook remains subject to risks including weaker discretionary healthcare demand, foreign-exchange movements, tariffs and trade disputes, geopolitical disruption, competitive pressure, shifts toward lower-priced products or deferred-revenue models, supply-chain constraints, regulatory requirements and changes in customer utilization. Align also expects fourth-quarter restructuring and related charges associated with lease, contract and employment terminations.
